How to Apply

Program Eligibility

  • Doctor of Pharmacy professional degree from an ACPE-accredited school
  • Florida pharmacist license within 120 days of program start date
  • United States citizenship or permanent residence (Green Card)

Application Requirements

  • Official transcripts from all colleges of pharmacy attended
  • Current curriculum vitae (CV)
  • Personal letter of intent
  • Three references utilizing the standardized reference form in PhORCAS (minimum three (3) narrative sections). One reference must be from a clinical preceptor.
